What Is The Cost Of Shingles Shot – WASHINGTON — In an unusually close vote, the Disease Control and Prevention Advisory Panel on Wednesday recommended using a new vaccine to prevent shingles over an older vaccine that was considered less effective.
The decision came days after the Food and Drug Administration announced approval of a new vaccine for adults 50 and older, called Shingrix and manufactured by GlaxoSmithKline. The panel’s recommendation favors the new vaccine over Merck’s Zostavax, which has been the only shingles vaccine on the market for more than a decade and was recommended for people 60 and older.

The Advisory Committee on Immunization Practices also recommended that adults who received the old vaccine receive the new vaccine. Despite the committee’s vote, the recommendation still awaits formal approval by the head of the CDC, which usually takes a few months. Insurers must also agree to cover the cost of the vaccine, which GSK estimates at $280 for two doses.

Once a person has chickenpox, the virus remains dormant in nerve tissue. Years later, it can be reactivated as a shingle. The CDC estimates that about one million cases are diagnosed in the United States each year.

Given a single dose, Zostavax showed a 51 percent reduction in shingles and a 67 percent reduction in nerve pain. Shingrix is given in two doses, and the company said clinical studies have shown it to be about 98 percent effective for one year and about 85 percent effective for three years.
By preventing shingles, the vaccine dramatically reduces the overall incidence of severe nerve pain, a permanent complication for one in three people who suffer from shingles. GlaxoSmithKline said it has tested the vaccine in more than 38,000 people.

“We believe Shingrix will inspire confidence in the protection one can expect from a shingles vaccine,” said Luc DeBruyen, company president of Global Vaccines.
The advisory committee’s recommendation would be considered an endorsement of Shingrix over Zostavax, although the committee’s closer vote, 8 to 7, could soften market losses for Merck.
Dr. Kathleen Dooling, medical officer of the CDC’s Division of Viral Diseases, said she expects the agency’s final recommendation to be released early next year.
“The Shingrix vaccine has the potential to prevent thousands of cases of shingles and its complications,” Dr. dooling.

However, she cautioned, more people had side effects on Shingrix than Zostavax, including fever and muscle aches.
“Patients and caregivers should be aware that this vaccine is very effective, but it causes more reactions than those used with other adult vaccines,” she said. “Everything indicates that this is not a health risk, but it can disrupt your daily activities for a few days.”
GlaxoSmithKline said the new vaccine will cost about $280 and will be available next month. Zostavax costs about $223.

New research shows that the number of seniors receiving the shingles vaccine is declining as seniors’ out-of-pocket costs for the vaccine increase.
Specifically, experts found a 40% to 60% increase in shingles vaccine adoption among enrollees in Part D plans offering $0 cost-sharing from 2012 to 2016. Overall, less than 10% of Part D enrollees in a plan received the shingles vaccine. $0 cost-sharing, while all other enrollees paid an average of 40% of the vaccine cost out of pocket. In 2016, 95% of Part D beneficiaries experienced cost-sharing for the shingles vaccine, paying an average of $85 out of pocket.
Research shows that vaccination rates for vaccines covered by Medicare Part B are significantly higher than for vaccines covered by Part D. For example, in 2016, 67% of non-institutionalized adults aged 65 and older received the pneumococcal vaccine, which is covered under Part B. While only 34% of adults age 65 and older received the shingles (or herpes zoster) vaccine, which falls under Part D.

Cost-sharing requirements for vaccines vary by market. Differences in patient cost-sharing obligations explain the significant difference in immunization rates between Parts B and D. Congress mandated coverage of influenza, pneumococcus, and hepatitis B (for intermediate- and high-risk populations) with restrictions under Part B. Share the cost. Under the Medicare Modernization Act (MMA) passed in 2003, all other commercially available vaccines must be covered under Part D. The MMA does not prohibit sponsors of Part D plans from requiring patient cost-sharing for vaccines. Like Part B, vaccines covered by commercial health coverage under the Affordable Care Act must legally be offered at $0 cost-sharing.
Part D beneficiaries face significant cost sharing. A 2016 report found that beneficiary cost-sharing for Part D vaccines in 2015 was between $42 and $54 in Medicare Advantage prescription drugs (MA-PDs) and between $14 and $102 in Part D stand-alone drug plans (PDPs). . . Cost-sharing has previously been identified as a barrier to patients receiving recommended vaccines. Other factors may also contribute to lower rates of Part D immunization compared to Part B, including physician billing and reimbursement issues.
In the CY 2016, CY 2017, and CY 2019 Advance Notices and Call Letters, which provide subregulatory guidance for sponsors of Part D plans, the Centers for Medicare and Medicaid Services (CMS) encouraged Part D plans to offer the vaccines. $0 to place share level.
